Event Request Form

This form will be submitted to the church office. All requests will be coordinated with the Church Staff. Requests are honored on a first come, first served basis. Dates will not be honored until this form has been completed fully and approved by the Church Staff. Events which require setup and takedown must have an Event Setup Diagram provided to the Church Office.

  • *Please let us know what type of set-up you need, even if you do not need assistance from staff. This helps us determine the best room for you.
  • *Podium, Sanctuary TV, etc. Please note that requests may incur an additional charge.
