Event Request Form
This form will be submitted to the church office. All requests will be coordinated with the Church Staff. Requests are honored on a first come, first served basis. Dates will not be honored until this form has been completed fully and approved by the Church Staff. Events which require setup and takedown must have an Event Setup Diagram provided to the Church Office.
NOTE: All users of the facilities, both members and non-members, must agree to and conform to the Building Use Policy. Available for download for Recurring Events or Single Events or visit the Building Use Page below:
